Introduction
This assignment will show what services are available in early year’s settings and for young people to support positive development. It will show what assessments we carry out within my setting and I will also critically analyse agencies that are available for referrals, transitions and safeguarding for children and young people.
Explain different theorist and frameworks of child and young person development.

Outcome based services available for children and young people
Health visitor
Health Visitors are specialists in child health and development who work at community family and individual level to promote and improve health and wellbeing of pre-school children and their families, a Health visitors work closely with midwives, practice nurses and GP’s. Health visitors are a qualified nurse or midwife who has gone on to do further training. Health Visitors provide an essential service to Children and Families and work within local communities to promote and improve health and reduce health inequalities. Child Health Practitioners and Nursery Practitioners support the Health Visitor in delivering this service.
Health visitors will offer advice and support on the following:-
Breast feeding support/advice
Advice on healthy eating and exercise
Child developmental reviews
Advice regarding child behavior
Support children with any additional needs
Support in safeguarding children
Pre-School immunisations advice
Referrals to child specialist services
Physical and Emotional Well Being for the family

Nursery practitioners within my setting are also able to give the above advice as best to their knowledge but will also recommend that parents and carers seek further advice from their Health visitor if they need to do so. Health visitors are
