Child Function Report
1. Please describe the child's activities during a typical day, from the time he or she gets up until bedtime, including any problems with sleeping.
I would usually wake Sarah and her brother at 7:00am during the school week. She would cry and say; "I hate school, school is bad". Everyday I would reply by asking:" Why do you hate school? She replies: Because I'm scared. Then I would ask: Why are you scared? She reply's: "because school is bad. Every morning we would start the day with those sentences.
She would then go potty and then I would remind her to wash her hands and brush her teeth. I usually put out breakfast for Sarah and her baby brother Christian at about 7:45am. Then at about 8:15am, we would walk to the school bus stop.
As soon as we are at the school bus stop, she would ask: "Do you wanna use my can opener, for this jar of mayonnaise? And phone books? I usually just play along so that the other kids at the bus stop wouldn't laugh at her.
When we see the bus coming down the street, I would tell her to get on line. She usually asks, "Where is the line mommy? She will only get on line when it is already formed. Sarah doesn't ever start the school bus line.
3:30pm
I pick her up at the bus stop, she is usually very glad to see me and her brother. We would start walking home, then on the way we stop by the leasing office for the cookies. Then we arrive home and fix Sarah and her brother a snack.
4:30
Sarah and I would do homework. I usually have to explain it to her in a way that she could understand, ex:
4+5=9
I would have to tell her to draw four little circles and five more circles. She would then say, 9? Then I would say, that's your answer honey!
5:00pm
Sarah and Christian get to watch TV while I make dinner. Around this time, Sarah's dad arrives...
